Sending case

– Send a Traditional Impression

1- Check the impressions for quality and accuracy. In order to ensure your safety, please pack your sanitized impression and bite in a biohazard bag, or a zip-top baggie. Please wrap models in bubble wrap before placing them in a bag or they will break during transit. Please also put small parts in an envelope or vial.

2- Please complete and print the prescription form online. Don’t forget material choice and shade! Please attach the prescription to the OUTSIDE of the biohazard bag.

If you have any files like photos please attach them to the order online.

3- After submitting online order Art & Cosmetic Dental Lab will receive a text notification, you don’t need to call to seclude a pick-up. We will pick up the case ASAP.

– Send a Digital Impression

1- Please complete the prescription form online. Don’t forget to select the material and shade.

Please attach Digital Impression including any files like photos.

2- After submitting online order Art & Cosmetic Dental Lab will receive a text notification so your case will be ready to start in the lab.
